If OTP is failing, check the Key, Time step, Code size, and Hash algorithm first. These settings must match the source system exactly.
A mismatched time step is one of the most common causes of OTP failure. Verify that the client device's clock is correctly synchronized before checking other settings.
Re-enter the OTP Key or load it from the QR image.
Confirm the Time step matches the provider in Properties – Advanced of the OTP entry.
Verify the Code size and Hash algorithm in Properties – Advanced of the OTP entry.
If you use an entry, make sure its Source, located in Properties – One-time password, is set correctly:
Linked (vault) for the preferred shared OTP entry.
Inherited only if every parent folder also uses Inherited.
None means no OTP is attached.
